Bring frus:attachment heads to the level and structure of 1981-88 volumes for ease of possible future metadata extraction to solve issue of "hidden" documents. - Spotted when reviewing attachments
- Several “empty” footnotes were valid. When footnote text was missing in original print volume, I added an XML comment to this effect. (How to communicate this to the reader though?) When multiple footnote numbers/symbols pointed to the same footnote, we use <note target="#fn-identifier">, without repeating the original @n or text. (Need to check if ODD handles this correctly.)
- Moved pb elements outside of otherwise empty note elements
empty notes content added via page image check per @joewiz's catch

@joewiz also noted 3 empty <hi/> tags in notes that indicated missing italicized titles of books--fixed as well.
