Active-condition replay on reconnect

Add condition_replay_strategy (method/read/auto/off). On ConditionRefresh rejection, browse the alarm sources, read ActiveState/Retain, drive the condition state machine, and reconcile the fault set on (re)subscribe.

• Acceptance: trigger an alarm, restart the gateway, the active alarm is reported again without a new transition.
