feat: make constructor class for feature extractor index

Used to take the mit_{DATASET}_index.csv or mit_{DATASET}_index-simple.csv and construct whatever input index is desired.

Examples:

• PyRadiomics needs ID, Image, Mask
• FMCIB needs image_path, coordX, coordY, coordZ
